Terms and conditions for $210 Per Week - Post Challenge - Bills Every 4 Weeks

Please read and sign below:
(a) I agree to purchase the 6 Month Billed Weekly Contract for $120.00 per week, as an automatic charge to my credit card account every 4 weeks for a contract period of 6 months;
(b) I hereby certify that I am the holder of the credit card;
(c) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card within 10 calendar days of the original rejection date;
(d) I understand that my service will be deactivated if my account becomes more than 30 calendar days late; and
(e) I understand that I will be charged a $10 Late Cancellation fee when I take a spot on a class roster but fail to show up.
(g) I understand that WCCF has an easy divorce policy. Should I wish to cancel my membership at any time I will submit the Request To Cancel form on the WCCF website and provide 30 days notice. I will also do a courtesy exit interview with Beth or one of the WCCF coaches
